Note that the formula Si Ni SAN is very different than the formula SI NI TANG. This is because the symptom of cold hands and feet (frigid extremities) can have several distinct causes, implying totally different treatments. One common cause of these symptoms is a tightening of the Qi of the chest, which constrains the flow of Qi and Blood to the arms and hands. This is called Liver Qi Stagnation. It is addressed by this formula, SI NI SAN (frigid extremities POWDER). Another cause of cold hands is internal cold, and is addressed by the formula SI NI TANG (frigid extremities DECOCTION).

Note that the original text states that this condition is due to 'hot type collapse YANG JUE, where heat enters the interior interfering with the spread of YANG QI to the extremities. It was later on that physicians began to use it for emotional constraint caused by Liver Qi Stagnation.
